Online Coaching Programs

 

Online coaching is an efficient way to train on your own time (gym, outside, home, etc.) but have a professional trainer and coach with you every step of the way.

Disclaimer: Regular physical activity is becoming increasingly popular as people of all shapes, sizes and abilities realize the benefits of leading healthy and productive lives. Being active should be fun, safe and a positive experience. But, there are heath risks to consider—injuries and medical. Before embarking on this journey, please check with your doctor before working with a trainer and becoming more active.

Lets get started. Please read the questions carefully and answer honestly.

Has your doctor previously informed that you have a heart condition and/or you should only do physical activity recommended by a doctor? *

Are you taking any medication for heart disease, high blood pressure, or type II diabetes that you didn't have to take when you were younger? *
Have you ever lost your balance due to dizziness and/or have you ever lost consciousness? *
Do you have a history of bone or joint problems that could be made worse by a change in your physical activity? *
Do you know of any other reason why you should not do physical activity? *

Any recommendation for changes in diet including the use of food supplements, weight reduction and/or body building enhancement products are entirely your responsibility and you should consult a physician prior to undergoing any dietary or food supplement changes. You agree that you are voluntarily participating in these activities and use of these facilities and premises and assume all risks of injury, illness, or death. By signing this waiver you agree to utilize this facility at your own risk. We are also not responsible for any loss of your personal property. You acknowledge that you have carefully read this “waiver and release” and fully understand that it is a release of liability. You expressly agree to release and discharge the trainer or instructor and/or Mario Flaherty Fitness & The Flaherty Effect from any and all claims or causes of action and you agree to voluntarily give up or waive any right that you may otherwise have to bring a legal action against the trainer or instructor and/or Mario Flaherty Fitness or The Flaherty Effect for personal injury or property damage. To the extent that statute or case law does not prohibit releases for negligence, this release is also for negligence. If any portion of this release from liability shall be deemed by a Court of competent jurisdiction to be invalid, then the remainder of this release from liability shall remain in full force and effect and the offending provision or provisions severed here from.
